We’re looking forward to providing you with great healthcare. You’ll save time on your first visit if you take a few minutes to gather the items you’ll need for your first appointment. Please print out the attached New Patient Registration Form and bring it filled out to your first appointment. Also, you will need your Insurance card, Picture I. D., copies of pertinent diagnostic tests and medical records, as well as a list of all current medications.
